Energy control form did not match the controls performed. Issue was corrected.
Potential Risk Severity :
Crew was not following social distancing policy correctly. Mask is needed anytime two or more come within six feet of each other..... even sitting in a truck.
PIC had green cone out marking powerlines but there was another power pole in the area that was unmarked. Issue corrected.
We are doing a Pigging job :Location nice and clean walking surface nice and clean people who we are working they know what they doing ,, everything went right, and we when home alive and well..
I observed nothing wrong on this job while we was pumping hydrogen peroxide, lot of fume comes out but we was but 100 feet’s away from it , Temperature was 84 so we was drinking plenty of water,job done safely
Vac truck, unloading water in to our pump truck, did not hooked up a static line. Until I told him
On this job we was about ready to open containment and inside of exclusion zone we were three people and only two were supposed to be allow inside , we had to take action and talk to the this people ,go out of the Exclusion zone
Driving back from Long Beach it was nice no traffic around 2:00 pm When used to be a lot traffic.

We was rigging up on a producer well , and one of the coworkers was bleeding off the pressure and he didn’t pay attention that I was walking by and he open the bleeder Little valve, Lucky the welldidn’t have any pressure.
SSE it was starting walking down on the cellar and I have to stop him and give him my quad monitor just in case there was any H2S
Overall the job starting Riggen up hook up acid ,pumping job and rig down I see transport driver Wearing all PPE
Doing producer acid dump, safety people doing observation and walking with out gloves and with out safety glasses
Pump Operaror was coming up and down from truck , two times I see him using the 3 points contact,, good job

While we was pumping the second job the company man was inside off the pump truck so someone came over to talk to him so my pumper had to stop pumping to get the company guy out and wait for him totalk to the visitor and then continue pumping the job that was Safety way to do it
While we was reading up on the first well after we have a safety meeting I told my coworkers don’t hammer without saying :: Hammer time :: and one of them still forgot it
Spotting equipment, trucks are picking up lot of dust, that I have to start wearing a dust mask .

while I was spotting A vacuum truck I got in between the pump truck and the vacuum truck moving at that moment imy safety supervisor told me to move off , that was crash point , I’ll thanks to mysupervisor to let me know not to be in between Both trucks
While pumping on a coil tubing high pressure hose started leaking on the connection to the coil and coworker approach the leak he was going to hit it with a hammer and I told the operator to stoppumping close the valve release the pressure and then hit the hammer union
While we was Rigging up connecting hoses my coworker start hammering and the other coworker was right next to it and I say stop move off always say hammertime.
while I was driving my supervisor stopped me because I was driving with the tailgate down . And the possibility was Chush the tailgate with shower trailer. Thanks to him didn’t happen.
Pumper understood exactly what I was talking about when I expressed why we do the "kick-out" on the pump.
In san Vicente Lese, while we were switching tanks On the street it was a lots of traffic so we have to get someone else from E&b company To help us to stop the traffic after all we did a safeentrance to location
PIC was a little unfamiliar with ball-out job. I double-checked to make sure all had a program. Not all did. PIC gave everyone a program.
Saw from a distance...… our Driver exited semi-truck facing forward without 3-point contact. Issue was NOT corrected.
I see one of my coworkers didn’t want to go down to the cellur and I asked him why and he told me because I didn’t put the Quad monitor yet ,,, good catch ..
Driver forgot to put down chalks for semi truck. No one noticed during job. I told PIC and issue was corrected.
PIC left keys in ignition. Also PIC didn't apply the emergency brake while out of vehicle.
This is a Rig cup wash , we was ready to unload hoses and the vac- truck driver it was trying to help us . I told him not to help us , just in case he gets hurt I talk to him nicely

While we was doing this pigging job, I know it that the hammer union that they was using it was a real sharpie and I told them to either grind it off or replace it . Their answer was agree .
My coworker was ringing up without setting wheel chocks on the truck,
Category may be off...…… Driver was working and had forgotten to chalk his tires on semi truck. EE corrected.
Working at night time, wind start blowing very hard it was real dusty. We start using the dust mask.
EE started working by dropping suction hoses before signing onto JSA. PIC corrected the issue.
Real tight location :: Truck driver wasn’t able to get the truck out of the location.PIC jump in the truck and backed the truck out safely.
While we was Pumping HF Another company came through and started walking passing the red tape. My co- worker call me on the radio and I came out and talk to them about the hazards. Pressure andchemicals
MTS Crew was rigging up on a disposal well and was having trouble removing the plug as it was super tight. It was as if Kyle was previously there and put all his strength into tightening it. Two pipewrenches were earlier used and one was at rest on top of the 24" flange. The 2nd pipe wrench was placed atop of the 1st wrench all fell straight to the ground. No one was hurt or injured but it wasstill a NEAR-MISS as the flange is about 6 ft. off the ground.
Survey company left their equipment onsite. We secured the location with signs and cones and still the survey employee walked into our site before waiting to be signed in. PIC spoke to him aboutwaiting next time!
I have my location secure,, stop signs , cones, just waiting for the pump truck and C&J tool truck just drive up in the middle.. I have to go talk to the guys, no big deal yet but soones my pump gethere, you need to be leaving..
On a rig job, one of the EE's from the company we were working with forgot to wear his eye wear after lunch. He was working for at least 10 minutes before I pointed out the mishap. Problem wascorrected.
PIC set up blower/ fan toward the pump truck as "open containment" was occurring. Coached PIC of Aera's process.
EE was not wearing side shields on eye glasses. Issue was corrected onsite.
EE tried to tighten bolts on leaking well by himself. EE asked for the snipe and another person to help pull. By doing so, leak was fixed (temporarily).
Hose Hobble shackle was the wrong size removed and got correct size
Park trucks next to the seller overnight can be a risk if it’s a leak on Wells under the cellars
Monitor wasn't calibrated
When I was using the shop dolly to move some stuff, I caught my self walking backwards and almost trip over a jack stand.
