MTS Crew was rigging up on a disposal well and was having trouble removing the plug as it was super tight. It was as if Kyle was previously there and put all his strength into tightening it. Two pipewrenches were earlier used and one was at rest on top of the 24" flange. The 2nd pipe wrench was placed atop of the 1st wrench all fell straight to the ground. No one was hurt or injured but it wasstill a NEAR-MISS as the flange is about 6 ft. off the ground.

Survey company left their equipment onsite. We secured the location with signs and cones and still the survey employee walked into our site before waiting to be signed in. PIC spoke to him aboutwaiting next time!
I have my location secure,, stop signs , cones, just waiting for the pump truck and C&J tool truck just drive up in the middle.. I have to go talk to the guys, no big deal yet but soones my pump gethere, you need to be leaving..
On a rig job, one of the EE's from the company we were working with forgot to wear his eye wear after lunch. He was working for at least 10 minutes before I pointed out the mishap. Problem wascorrected.
PIC set up blower/ fan toward the pump truck as "open containment" was occurring. Coached PIC of Aera's process.
EE was not wearing side shields on eye glasses. Issue was corrected onsite.
EE tried to tighten bolts on leaking well by himself. EE asked for the snipe and another person to help pull. By doing so, leak was fixed (temporarily).
Hose Hobble shackle was the wrong size removed and got correct size
Park trucks next to the seller overnight can be a risk if it’s a leak on Wells under the cellars
Monitor wasn't calibrated
When I was using the shop dolly to move some stuff, I caught my self walking backwards and almost trip over a jack stand.
